What is Hospice Care?
- Hospice care is a specialized care and support program that focuses on maintaining the qaulity of life for people who are in the last phases of their incurable or life limiting illness, so that they may live as fully and comfortably as possible.
- Hospice care not only supports the patient, their families and
caregivers can get support as well. - Hospice focuses on providing comfort and symptom management at home.
Who pays for hospice?
- Hospice is a benefit fully covered through Medicare and Medicaid/ Medi-Cal.
- Many HMO/ private insurance companies provide coverage for hospice care as well like IEHP, Molina, LA Care Healthnet, Blueshield Healthplan, and other HMO insurances.
- If unsure, if your insurance covers hospice, please do not hesitate to call us.
Where is hospice care provided?
- Our services are provided wherever you or a loved one calls home, whether it is a private residence, assisted living facility, board and care or
nursing home.
Hospice also provides:
- Medications related to terminal diagnosis, comfort and symptom
management - Durable medical equipment such as, oxygen concentrator, hospital bed, nebulizer, walker etc.
- Medical, incontinence and wound care supplies.
What services does hospice offer?
- Hospice Physician- directs and oversees the plan of care
- Hospice Nurses- provides symptom management and educates the family skills to help patient be more comfortable
- Certified Home health aides assists in activities of daily
living, such as shower and Bathing. - Chaplain services- assists in providing spiritual needs at
home. - Social Worker- assists by making referrals to community resources and legal paperwork.
- Bereavement and counseling
- Hospice Volunteers- provide companionship for the patient
